I would bet he was holding a 3" mortar tube when it happened.
I know a guy in the ithaca area who did that about 4 years ago and blew the skin and bones from a few fingers right off. I the photos of it and it was down right nasty.
You need to let the tubes to fully cool and if you don't the mortars will blow up inside. Most will give you 2 tubes for that reason.
Then you have the young man(22) put one on his head and killed him self instantly.
