Bring your COVID19 vaccine card to your family doctor and ask them to upload your information into the New York State Immunization Information System (NYSIIS).  I was told it will take about 2 weeks for NYS to upload your information onto the Excelsior Pass website.  Below are the details:

You will submit these as historical doses to reflect that you did not administer the dose to the patient.
