I don't think it happens too often, now with slotted money and revenue sharing all these teams have money to sign their picks.

In 2014 the Houston Astros pulled out of a deal to sign the first overall pick and as a result the Astros got the 2nd overall pick last year.

If you don't sign your first round pick you get the compensatory pick one slot after the following year.
