If I was relevant in his life and could be frank. Driver and rehab is obvious, but the underlying issue here needs to be fixed... and its not the pain itself, its what is causing the pain. His constant attempts to comeback are physically intense and his body can't take it.. he needs to accept he is just a ceremonial golfer at this point.

#1. Get a driver
- You are damn lucky you haven't hurt yourself even more in accidents, and even more lucky that you haven't killed others. You are lucky you are not in jail.
I can have sympathy for someone that has a drug dependency issue due to pain. But a drug dependency issue is never an excuse for impaired driving - no sympathy for that.
#2. Go to rehab
- Its obvious you have a drug dependency issue which was caused (or continues to be caused) as a way of managing pain
- Get help for that dependency and go to #3.
#3. What is the underlying issue here? What is the plan.
- What is causing all this pain? To me the cause of the pain is his maniacal workouts and attempts to come back which puts an even further strain on the back.
- Accept 2019 was the high point comeback, and your body just can't handle you maniac like training and attempts to be a top level pro golfer again. Accept you are a ceremonial golfer and find other things in your life.
