It wasn't a great pass, on 1st down, running, with 2 guys about to sack him. The pass was almost the equivalent of just throwing the ball up on a hail mary. The fact that Williams didn't make a catch on a ball thrown behind him to two defenders is an asinine assessment of what happened. The only part of this that involves Williams is he just so happened to be in the location of where the ball was being thrown. There were many reasons that you don't make that throw given the exact circumstances, to blame the WR is silly.
So the biggest question is was Dungey ever great in the passing game? I remember a lot of plays where he did just chuck it up to either Etta or Ish and they would win the battle down the field. Occasionally he will make great throws but overall he isn't a consistent accurate passer needed for this system. He is a warrior, leader and playmaker but isn't an accurate passer especially down the field especially in this offense.
Location wasn’t great for ED but that’s what happens when you’re basically falling to the ground with two Pitt guys on ya. Maybe I am not recalling that correctly but pretty sure he wasn’t standing there clean and misfired...pretty sure he was getting drilled as he threw it.

We start the final OT and rush for a TD (albeit in one wonderful play). VT, on first down just throws one up in double coverage, and it's intercepted. Probably the only bad thing their QB did all game. I think he just didn't want to play anymore.
I'm pretty sure the right play was to throw that ball out of bounds and try again 2nd and 10. We only had two WRs run routes and both were doubled. Even if he completed the pass, it was risky at best - 30 yards into double coverage. It wasn't like the clock was at :00 and this was the last play no matter what. If he’s playing with a shoulder injury (which surprise actually no one given his history and the way he slam a husband shoulder into LBs And DLs) and we keep running him out there...Plus would not be surprised if he's carrying a shoulder injury on his throwing arm as others have speculated... and if that is the case, it would make that throw even more of a head scratcher...

Johnson was split left and jogged out about 3 steps and stood there next to his CB. Everyone else rolled right.Well, the best play would have been to throw it back the other way to the Tight End who snuck out to the left.
Had we actually tried that.
I know where to find one...offense doesn't work if you can't throw over the top at all

[edit my edit]After watching the replay about 10 times in slomo, my initial take is wrong and I admit my memory sucks. The play was open if we had been able to block and let ED step into the throw. Williams was behind his defender with the safety well back in pursuit. The pass blocking was completely and utterly destroyed so that ED was pushed backward on the roll out and then hit during throwing leading to a throw that was well short and behind.

at that point, we need to go for two - resting the defense wasn't even a consideration to meWhat bugs me is this: the defense had been on the field a prolonged time at the end of regulation. We win the coin toss and elect to send them right back out there for OT. OK, I understand it's highly desirable to have last ups in OT. So Pitt marches back into the endzone with little resistance, and our defense now seems even more gassed. On our first play of the next possession, why even consider a quick strike TD? As others have noted, the smart move was to throw the ball away and live to fight another down.
We needed to buy the defense a breather - unless we were planning to roll the dice on a 2 point conversion to end the game. Had we kicked an extra point and sent the tired defense right back on the field it seems our chances for success would have been mighty low.
Anyone think we would have gone for a 2-pointer if the pass had been completed? Maybe a good question to throw at coach on the next call-in show.
