If the officials deem that it was ripped off by an opponent, pretty sure I've seen that they can allow you to stay in the game and call a penalty on the opposing player that caused it.

I learned this watching a Green Bay game years ago if the ball is on the sideline the player should go out of bounds then touch the ball. It’s makes the ball

Agreed. Happens in both football and lacrosse. Guys like to wear them loose. Whether it's more comfortable or just "cool", it happens and the rule is supposed to deter it. Coaches don't like when you have to leave the field because you don't want to tighten your helmet.
